Terms and Conditions
Welcome to Divinity Aesthetics! These terms and conditions outline the rules and regulations for the use of our website. By accessing this website, we assume you accept these terms and conditions, in full. Do not continue to use this website if you do not accept all of the terms and conditions stated on this page.
Appointments
To ensure that you get an appointment for the date and time you desire, we suggest booking a few weeks in advance. Our website booking link provides the most up-to-date availability and convenient self-scheduling. Please note, we require a credit card to reserve your appointment. We accept American Express, Discover, MasterCard and Visa.
Cancellations
For non-group appointments, please provide a minimum of 24 hours notice should you need to cancel or reschedule an appointment. You will be charged a nonrefundable $100 fee if an appointment is canceled or rescheduled less than 24 hours in advance, or the appointment is marked as a no-show. If you arrive more than 10 minutes late for your appointment, you may be required to reschedule to avoid disrupting other clients’ appointments. There are no exceptions to these policies.
Payment Options / Pricing & Prepayments
Payment is required at time of service. Divinity Aesthetics provides elective, cosmetic procedures which are not deemed necessary for health maintenance. As such, we do not participate in medical insurance plan coverage and all services are self-pay. Prepayment for services may be required in certain circumstances and when applicable, will be communicated to patient, in advance. Promotional packages must be paid in full at time of the first scheduled treatment to receive special pricing applied.
A credit card is required for appointment reservation and cancellation penalties will be applied, as previously outlined.
THERE WILL BE NO REFUNDS FOR SERVICES ALREADY PROVIDED.
In the event that a package or series of treatments has begun, services will be considered to have been rendered even though the full series may not have been completed. Should you wish to discontinue your treatment in the midst of a series, credit for the pro rata share of unused treatments at the discounted package price may be extended and may be used to purchase other treatments or products offered by Divinity Aesthetics. Transfer of credit to another individual for use in exchange for treatments or products of comparable value to the credit will be considered on an individual basis. Financial policies are subject to change without notice. If you have any questions or need assistance with any financial matters relating to your treatment, please contact our office for help.
Consultations
Basic consultations are available at no cost, with more extensive assessment and consultation requiring an appointment deposit. We believe an informed client with realistic expectations will be most satisfied with our treatments and results. Our consultations include education about your unique skin type, a comprehensive discussion about all options available to you, both immediate and long term, and a clear plan of action based on your individual goals. There is a limit of 2 free consultations, per patient, per 365 days.
Film & Photo Policy
All photos and videos taken as part of treatment documentation are, and will remain, 100% sole property of Divinity Aesthetics. Photos taken for purpose of before / after comparison may be shared with other clinic patients, to demonstrate specific anatomy, anticipated treatment outcomes, etc. Clients, or a friend of a client, are prohibited from filming, recording, or taking photos of a Master Provider or Master Aesthetician in a treatment room or anywhere on property. If you would like to film, please seek written approval from Divinity’s Marketing Team or Practice Manager.
Treatment and Treatment of Complications
The practice of aesthetic medicine is not an exact science, and services provided by our healthcare providers fall under that umbrella. While excellent results are anticipated, there can be no guarantee or warranty, expressed or implied, by anyone as to the actual results you may receive. Occasionally, additional treatments and / or treatment for problems or complications may be required. These may result in additional charges for which you may be responsible. Your insurance, if you have it, may or may not cover the expenses related to actual complications or other medically related problems arising out of treatment at Divinity Aesthetics.
Proceeding with treatment / acceptance of services provided, implies acknowledgement and understanding of all content herein contained.
Child Policy
To ensure the safety of children and the enjoyment of all clients, we ask that parents or guardians make other arrangements for children while receiving their services. Children who visit Divinity during their parents’ appointments, or children who are unattended in the waiting area, can pose a health and safety risk due to the nature of our business. An adult must accompany children under the age of 18 receiving services. Please understand that our concern is for the safety of your child. We hope you will aid us in maintaining a safe and comfortable clinical environment. Don’t hesitate to get in touch with us if you have concerns or questions about this policy.
Pet Policy
For the health and safety of our clients, Divinity has a no-pets policy. Although we love animals, we ask that you please leave your pet at home during your visit to Divinity Aesthetics. Only working service dogs are permitted.
This no-pets policy applies to:
Divinity Aesthetics complies with the Americans with Disabilities Act (ADA), allowing access for all individuals to public places; therefore, we allow working service dogs to accompany our patients. Service animals are individually trained to perform work or tasks for people with disabilities. Service animals must be leashed or harnessed except when performing work or tasks where such tethering would interfere with the dog’s ability to perform the work or tasks.
Dogs whose sole function is to provide comfort or emotional support do not qualify as service animals under the ADA. Under ADA regulations that became effective on March 15, 2011, there are no protections for emotional support animals in terms of access to public accommodations and public entities. The Department of Justice has stated that emotional support animals are not protected as service animals under these regulations.
Should you arrive to an appointment with a pet that is not a service animal, you will be asked to remove the animal from our offices. To avoid any disruption or inconvenience, we ask that you please leave your pet at home. Thank you for your cooperation and consideration of all our clients.
